This book adequate for to learn SOAP notes. It does show you how to break it down. However, it doesn't quite explain how to write an addendum or other aspect. The book could have had a better layout and more examples. This books is a workbook so as always practice and you'll learn it.

I would recommend this to any health care professional needing to learn or improve their soap note writing ability.
